For Tenure-Track and Tenured Faculty
Application Deadlines
Complete application packets must be submitted to the Graduate School by:
- Fall 2024: Friday, September 19, 2025, by 5:00 p.m.
- Spring 2025: Friday, March 13, 2025, by 5:00 p.m.
New Application Format - Full Graduate Faculty Status
Faculty applying for Full Graduate Faculty Status should use the updated application form:

When completing the form, please ensure the date range reflects the most recent five years. Additional guidance is provided below.
Application Requirements
Each applicant must submit a complete packet with all materials required for review by the Graduate Faculty Membership Committee.
Application materials should include:

Evidence of graduate teaching, advising, and mentoring during the most recent five-year period

Other relevant documentation as outlined in the Categories and Terms of Membership
Instructions for Scheduled Teaching Report
Export and attach a Scheduled Teaching report from the Success Portal covering the most recent five years:
- Access the Success Port through My福利小视频 or directly at
- Select Reports from the top menu>>then Scheduled Teaching
- For "Term Start," select five years prior; for "Term End," select the current term.
- Select Run Report
- Select Actions
- Select Download
- Download as PDF or RTF
- Depending on your web browser's settings, the file may open immediately or download. Save the report to your computer or OneDrive
Do not include CVs with the full status application.
Submission Instructions
Preferred Method - DocuSign
Submit your application through DocuSign (instructions). Please upload two files:
- Completed Graduate Faculty Application
- Scheduled Teaching Report (downloaded from the Success Portal)
Route the packet in the following order:
- Applicant - for signature
- Department Head - for signature
- Dean - for signature
- Rosalyn Martinez (rcmartinez) - CC only (receives a copy)
Alternative Method - Email
If you are unable to submit via DocuSign, email the fully signed application and the Scheduled Teaching Report to: rcmartinez@valdosta.edu
